At the reach in portion of the prep top pizza cooler the following parts are in need of repair: 1. Observed door seals to be cracked and open correct by replacing the seals. 2. Observed the inner door panel of the pizza dough compartment to have severe damage from impacting sheet trays. The damage has exposed the insulation inside the door to the food storage compartment. Correct by repairing the door

At the front service counter/front make area observed six unshielded fluorescent light bulbs. Correct by installing proper shielding for the light bulbs. This is a repeat violation

(Critical) At the cart holding the slicer, observed the following dirty food contact surfaces: 1. The food slicer with food debris, dead pests, and pest droppings 2. A storage container on the cart containing plastic wrap and wax paper. The container has dead pests and food debris. 3. A storage container with condiment pumps and utensils. The container has food debris and grime. Pic instructed staff to disassemble, wash, rinse and sanitize all of the affected items. Item corrected

In the kitchen and basement storage area observed several dead and dying cockroaches. An invoice for pest control service, dated 10/13/2014, was reviewed during the inspection. Correct by removing dead pests.

(Critical) Observed no date marking for ready to eat foods. Correct by properly date marking phf rte foods. Phf rte foods include meat, dairy or cooked pasta, held more than 24 hours and served cold. Once opened or prepared or sliced the food must be used or consumed within 7 days. For example a food opened today 10/11/2011 would be date marked for discard on 10/17/2011

(Critical) Observed several deli meat and egg salad in the raised rail section of the front line refrigerator at 48-50 degrees f. These items shall be stored 41 degrees f. Or less to reduce bacterial growth. Pic discarded affected foods items and placing the refrigerator out of service. Pic also called a refrigeration repair company.

(Critical) Observed shake mixing machine at front counter area with shake product dried on mixer blades. Pic could not identify when machine was last cleaned. Equipment used in preparation of phf must be properly washed, rinsed, and sanitized at least every four hours. Correct by wash rinsing and sanitizing at least every four hours or more frequently

(Critical) The cutting board on the main pizza make table is badly worn. Food contact surfaces must be easily cleanable. This is a repeat violation. Continuous violations require a risk control plan be submitted within ten days. Correct violation by repairing cutting board or replacing to provide a smooth cleanable food grade surface

The front service line refrigerator is not designed to provide 41 degrees f cold holding. Correct by upgrading or replacing unit with one capable of maintaining a temperature of 41 degrees f or less. Submit a manufacturers specification sheet documenting equipment meets this requirement

Health Report - 04/27/2011 (3 critical incident(s))

(Critical) I found three containers of cooked shredded chicken stored in covered containers, at a temperature of 48 degrees f. The chicken was prepared two days ago. As discussed, there is too much food in there containers. Secondly, never cover a container of food, during the rapid cooling process. Use one of the approved cooling methods listed above. Food temperatures must be taking during cooling to ensure that the rapid cooling parameters are met. I recommend using cooling charts. Corrected at the time of inspection. The food has been discarded.

(Critical) The interior of the microwave oven needs to be cleaned. Food contact surfaces must be frequently washed, rinsed and sanitized, to maintain good sanitation. Increase the cleaning frequency of the microwave oven. Corrected at the time of inspection. The microwave oven has been cleaned as directed.

(Critical) The cutting board on the main pizza make table is badly worn. As discussed, it must be replaced. Food contact surfaces must be easily cleanable. Badly scared and worn equipment must be promptly discarded. A follow-up inspection will be made to ensure that this critical violation is corrected.

I observed a hoodie stored on top of a flour container. Personal items, such as clothing, must be kept completely apart from the food service operation. This is to prevent food and work surfaces from being contaminated.

Chlorine test strips are not readily available. After a search they were found. Store the strips in a handy location, so they can be used.
